EMS Billing

At Butler County EMS we understand the financial difficulties that can come following emergency medical care. Our Business and Billing Office personnel do all that they can to assist patients through the process of insurance reimbursement and billing.

Medical Records Request

Butler County EMS accepts requests for copies of medical records / patient care reports from patients or the patient's legal representative. Requests for copies of medical records / patient care reports are accepted in person at our our administrative offices or online (see below).

Butler County EMS is diligent about the security of medical records and the privacy of all records associated with medical care and EMS operations. Security and privacy our always our first priority. Therefore, every procedure to ensure the security and privacy of our records will be followed whenever any request for records is received, regardless of the nature of the request. Request Records in Person or by Mail

In order to request a copy of your medical records in person or by mail, you must submit to BCEMS Form HF11: Patient Request for Access to medical records. Please print the form, complete it, and present the completed and signed form in person to BCEMS or by mail to:

Be sure to attach a clear photocopy of a valid photo ID of the individual requesting the records release. If you have any questions, please contact our administrative offices at (316) 321-9260.

Request Records by Phone

You may call our business office to make a request for medical records. However, we will still require that you submit form HF11. Our administrative staff is available to assist you with any questions, so feel free to contact us at (316) 321-9260 with your questions or concerns.

Requesting Records Online

The American Recovery and Reinvestment Act (ARRA), signed into law by President Obama on February 17, 2009, includes Title XIII, Health Information Technology for Economic and Clinical Health Act (HITECH), with the goal to provide Electronic Health Records (EHRs) for every person in the U.S. by 2014 leading to improved care and lower costs. Butler County EMS takes this goal very seriously and have implemented an electronic medical records solution for tracking your patient encounters with Butler County EMS. As required by law, we have implemented technical, physical, and administrative safeguards to ensure the availability, integrity, and security of your electronic protected healthcare information (ePHI).

As required by law, the link below provides you access to a copy of your medical record that we keep in electronic format. This site is only for the use of our patients and their authorized representatives. Unauthorized use of this site is strictly prohibited by federal law and punishable by criminal and civil penalties.

Please read all of the following carefully: You may utilize the link below to request online access to your BCEMS medical record(s), or BCEMS medical records to which you are legally entitled to have access. In order to streamline your request, be sure to include all of the information requested. A decision regarding your request will be made within three business days.

Please note: In some situations, additional information or documentation may be needed to process a request. If this is the case with your request, a Supplemental Information Request Form will be sent to you by email. If this occurs, you will still be able to complete the form, and your request, entirely online.
